SWMS Declaration

By ticking "I Agree" I confirm all of the following:


1. I have read the SWMS document shown on this page in full and understand its contents.


2. I understand the hazards, risks and control measures that apply to my scope of work on this project.


3. I agree to comply with all control measures in the SWMS for the duration of my works on this site.


4. I hold a valid White Card and any trade licences required to perform my scope of work.


5. If site conditions change or a new hazard arises not covered by this SWMS, I will stop work immediately and notify the Site Supervisor before resuming.


6. Failure to comply with this SWMS or any site safety instruction may result in immediate removal from site.


7. I am fit for duty — I am not fatigued, unwell, or under the influence of alcohol or drugs.


8. If my work involves after-hours shifts: shifts will not exceed 12 consecutive hours and I will take a 30-minute break after every 5 hours of work.

• Wear all PPE specified in the SWMS at all times

 

• Engineered stone / silica: WET CUTTING ONLY — dry cutting is prohibited

 

• After-hours shifts: maximum 12 consecutive hours | 30 min break per 5 hours
